Position Summary:
\n
The Community Health Navigator (CHN) will collaborate with providers on the USF College of Nursing mobile clinic to assist patients with identifying and accessing credible resources related to social needs. The CHN must have a strong passion for decreasing health inequity and assisting patients and families to achieve better health outcomes. The CHN will serve as a resource for education and support for mobile clinic patients and also network and develop relationships with area agencies that provide social support to Tampa Bay residents.Minimum Qualifications: This position requires a high school diploma or equivalent, with three years? experience in office, administrative, or customer services positions. Appropriate college coursework or vocational/technical training may substitute at an equivalent rate for the required experience.

Preferred Qualifications: Associate degree or higher. At least one year of experience in healthcare setting (for e.g., navigation, health education, or care coordination, etc.). Basic administrative and clerical skills. Basic Medical terminology use and understanding. Excellent verbal communication skills. Proficiency in word processing

20% Assess new and continuing patient needs on social determinants of health to identify and address any gaps in care. Collaborate with healthcare providers on the mobile clinic to develop care plans for patients that address social determinants of health.

20% Monitor progress, track outcomes and document patient and agency interactions

10% Build relationships with patients and families to promote understanding and trust. Build relationships with area agencies to promote understanding and trust.

5% Maintain up-to-date knowledge of community resources and healthcare services.
