👩‍⚕️ About the Role:
As an MDS Assistant, you'll be the right hand to our MDS Lead — helping to ensure that care assessments are thorough, timely, and compliant. You'll assist in gathering data, coordinating with interdisciplinary teams, and making sure our residents' needs are reflected accurately and completely in their care plans.

🛠 What You'll Be Doing:

• Supporting MDS assessments and documentation
• Assisting with care plan coordination
• Ensuring regulatory compliance and accuracy
• Collaborating with nursing and rehab teams
• Helping track quality measures and performance indicators

🎓 What You Bring:

• LVN or RN license (CA)
• Previous MDS experience or strong clinical documentation background
• Knowledge of PDPM and Medicare guidelines a plus
• Organized, detail-oriented, and team-focused
• A can-do attitude and genuine care for residents
