Full job description

Responsibilities

• Coordinate and oversee all functions of the MDS and resident care assessment process.
• Schedule and complete timely and accurate assessments to assure compliance with federal and state requirements.
• Monitor completion of MDS assessments by disciplines in addition to nursing and take follow up action as necessary.
• Conduct care plan conferences alongside the interdisciplinary care team for residents and responsible parties.
• Analyze Quality Indicator reports for the nursing department. Perform other duties as assigned by the Director of Nursing Service.

Skills

Knowledge of Patient Driven Payment Model (PDPM) and ICD 10 medical coding.
Familiarity with electronic health record systems Point Click Care.
Ability to work both independently and collaboratively with interdisciplinary team members and address resident/responsible parties' concerns/questions that may arise.
Keen attention to detail and accuracy in documentation.
Strong organizational skills to manage multiple assessments, deadlines, and meeting schedules.

Job Type: Full-time

Pay: $95,000 - $105,000
