Intake Coordinator (Administrative)
Our Intake Coordinator Will:
Conduct initial intake assessments for potential patients, gathering necessary information about their medical history, current condition and care needs. Provide applicable information to eligibility team for hospice and palliative referrals. Evaluates home health eligibility based on agency criteria and payer regulations. Effectively collaborate with communities, families, applicable stakeholders, and other team members to coordinate scheduling of start-of-care visits based on client's needs and staff availability. Serve as an initial point of contact for referral sources, responding promptly to referrals demonstrating professionalism, empathy, responsiveness and attention to detail. Record intake notes capturing all relevant information obtained during the intake process. Communicate pertinent information effectively with the clinical team. Be willing to seek out new methods and principles and incorporate them into existing practices. Accept change and adapt well with a positive attitude, while asking questions and providing constructive feedback.
