REGIONAL DIRECTOR OF SOCIAL SERVICES HOSPICE (Project Management)



Position Summary
The Regional Director of Social Services provides leadership, supervision, and accountability for hospice social workers across assigned regions. This role ensures the delivery of high-quality psychosocial support, patient advocacy, and family counseling as integral components of hospice care.

The Director participates in interdisciplinary team (IDT) meetings, verifying and validating attendance by the SW team, ensures compliance with CMS Conditions of Participation (CoPs), and holds social workers accountable for meeting documentation, patient satisfaction, and performance metrics. This leader fosters a culture of compassionate, patient- and family-centered support, while ensuring staff are aligned with organizational KPIs and regulatory standards. This position operates within the RACI model to ensure clear communication and transparency. This role will be responsible for additional duties and assignments as the Hospice Division develops.
Key Responsibilities
Leadership & Accountability
Provide supervision, coaching, and accountability for social workers across the assigned region.
Establish clear performance expectations and monitor staff compliance with KPIs, including timeliness, accuracy, and patient satisfaction.
Ensure social workers are fully integrated into hospice operations and collaborate with other disciplines.
Support staff development through mentorship, training, and professional growth opportunities.
Clinical & Interdisciplinary Integration
Actively participate in IDT meetings, ensuring psychosocial issues are identified, addressed, and documented.
Ensure care plans include appropriate psychosocial interventions and family support services.
Partner with nursing, chaplains, and volunteer/bereavement staff to ensure holistic care.
Patient & Family Support
Oversee delivery of psychosocial support, crisis intervention, and counseling for patients and families.
Ensure staff provide education on advance care planning, community resources, and patient/family rights.
Respond to escalated or complex patient/family needs as required.
Promote a culture of dignity, respect, and advocacy for patients and families.
Compliance & Quality
Ensure all documentation meets CMS CoPs, agency policy, and timeliness standards.
Partner with QAPI teams to integrate social services performance into quality improvement initiatives.
Monitor patient/family satisfaction surveys and implement strategies for improvement.
Ensure compliance with confidentiality, ethical practice, and legal/regulatory guidelines.
Bereavement Services (CMS Required)
Responsible for bereavement assessments and plans of care for families and caregivers, extending a minimum of 13 months after the patients death.
Responsible timely follow-up, counseling, support groups, and resource referrals for grieving families.
Monitor CAHPS and satisfaction data related to bereavement and implement strategies for improvement.
Collaboration & Communication
Collaborate with the Director of Spiritual Care and Director of Volunteers & Bereavement to provide comprehensive support to patients and families.
Maintain transparency and communication with executive and regional leadership regarding social service performance.
Represent social services in organizational meetings, trainings, and external partnerships as needed.
