MDS Coordinator (Healthcare)



Full Time Wage DOE: $40-$50/hour

Description

We are seeking a dedicated and knowledgeable MDS (Minimum Data Set) Coordinator to join our team at Colonial Vista Post Acute. As an essential member of our team, you will play a critical role in ensuring compliance with regulatory requirements and enhancing the quality of care for our residents or patients. The MDS Coordinator will manage the assessment process from start to finish, focusing on the accurate completion of the MDS assessments and submissions. This position requires a thorough understanding of the Resident Assessment Instrument (RAI) process, as well as familiarity with the state and federal regulations regarding long-term care. In addition to conducting assessments, you will also be responsible for training staff on MDS-related procedures and collaborating with interdisciplinary teams to ensure that all patient care plans are comprehensive and individualized. You will monitor and analyze data trends to identify areas for improvement in patient care and outcomes. Responsibilities

  • Conduct and oversee MDS assessments for residents
  • Ensure compliance with state and federal regulations

  • Prepare and submit MDS documentation accurately and timely

Requirements

  • Current nursing license or appropriate healthcare certification
  • Proven experience with MDS assessment and RAI process
  • Strong understanding of state and federal regulations governing long-term care
  • Excellent communication and interpersonal skills
  • Ability to work collaboratively in a team environment
  • Detail-oriented with strong organizational skills
  • Experience in patient care planning and quality improvement initiatives
